Microsoft Forms: How to Create and Share Forms with Microsoft 365


Introduction

With Microsoft Forms you can create online surveys, forms, quizzes and questionnaires within minutes!

This guide covers the steps necessary to create and share a poll/survey with Microsoft Forms. It also covers the different questions types and features available in Forms. 

 

Instructions

Getting Started:

Navigate to Microsoft Forms in your Microsoft 365 Apps, if you are having trouble locating it you can always search for it in the search bar of your Microsoft 365 home page.

1. Locate "Microsoft 365" at the top of your WesPortal:

You can also access Microsoft 365 via the direct URL: https://login.microsoftonline.com/?whr=wesleyan.edu  and sign-in with your Wesleyan SSO credentials.

2. From the Microsoft 365 Home Page locate Forms via the top left menu:

3.Once in Forms select the option to Create New Form
The difference between creating a new Form vs a new Quiz is that in Quizzes, you can add answers and scores, as well as math equations. You can also provide feedback on the user's selected answer - explaining why it is right or wrong.

Creating your new Form:

Creating a new form is quick and easy:

1. Click on "Untitled form" Create a title (name) for your Form and provide a description/introduction to your intended audience. 

example (note: if desired, you can add/upload a graphic/picture via the top right icon circled below):

*Pro tip: The name and description should be intuitive and informative to your intended audience (see example below.)

2. Add your questions:

You have the option to add questions as multiple choice, open text answers, rating scales, date options, and more:

Click the + sign to add more questions.

3. Preview the way your Form will appear to your audience at any time by selecting the Preview option from the top right toolbar:

Select your Form's Settings:

Click on the three dot ... overflow menu to choose your Form's settings:

Advanced Settings options, to including a form collection "start date" and "end date," are available:

Share your Form and Collect Responses:

Now that your Form is created, you can share it and collect responses by selecting "Collect responses" from the top menu bar:

You choose: who you wish to share the Form with, if an individual can respond more than once, and you have the option to shorten the URL for the Form:

Your Form is now ready to share via the URL! Copy the URL, send it to your desired audience and await the responses!

Collect and View Form Responses:

As the owner of the Form, you can receive email notifications when people respond and you can visit the form at anytime to view responses or even to edit the live form if you desire.

1. Simply navigate to Forms at anytime via the Getting Started instructions at the start of this article.

2. Select "My Forms" from your Forms home page:

3. Find and Click on the desired form

4. Open the Form and select "Responses" from the top left menu:

5. You have the option to view the responses and various Form analytics as well as to open and/or download the results in a Microsoft Excel file.
